Before you begin
SLS supports adults aged 18 and over whose assessed needs may be met by our supported housing and care services. Enquiries can come from the person themselves, a family member or an involved professional.
The process
The timing depends on assessment, suitable availability and funding approval. The referrals team will keep the people involved updated.

The referrals team reviews the information and arranges a conversation with the person and relevant professionals or supporters.
Where SLS can meet the person's needs, we discuss available services and arrange a visit before a decision is made.
The support plan, care hours, housing arrangement and funding approvals are confirmed with the responsible organisations.
A move-in plan is agreed, including dates, introductions, medication arrangements, personal belongings and any reasonable adjustments.
