For NHS and Council Professionals
Supported Living Services Ltd receives and assesses referrals from health and social-care professionals. Where a service is commissioned, responsibilities, outcomes, information sharing, review arrangements and price should be recorded in the relevant contract and support plan.
Current availability, staffing, regulated activity, property suitability and funding are checked for each referral. This page does not replace due diligence, a service specification or a placement agreement.
Care Pathways
Assessment considers the person’s goals, care and support needs, risks, preferences, funding and the suitability of any available service.
Any healthcare, therapy, education or other specialist input must be identified by provider and funding source. It should not be assumed to be included unless the placement documents say so.
Reviews should involve the person and relevant professionals, with changes agreed and recorded through the applicable care, support and commissioning process.
Safeguarding
Safeguarding concerns should be reported without delay and handled in line with the Care Act framework, local safeguarding arrangements and the person’s wishes and wellbeing.
Commissioners can ask for the current safeguarding policy, named operational lead, training matrix and recent assurance evidence as part of due diligence. See the public safeguarding page for reporting routes.
Working in Partnership
For each placement, the parties should agree roles, review frequency, escalation routes, information-sharing arrangements and evidence requirements. Claims about outcomes or performance should be supported by current service-level data.
Staff Training
Staff learning must be role-appropriate and recorded. Commissioners may request the current training matrix, completion records, refresher schedule and safer-recruitment evidence for the service under consideration.
