GP registration and appointments
Get help finding a GP surgery, completing registration steps and planning appointments, travel, communication needs and follow-up actions.
For residents
Wellbeing is personal. SLS can help residents understand their options, keep important appointments and build routines that work for them.

Call 999 or go to A&E if someone is in immediate danger. For urgent mental health support that is not an emergency, call NHS 111 and select the mental health option.
Everyday wellbeing
These are common areas where a resident may choose to include support in their personal plan.
Get help finding a GP surgery, completing registration steps and planning appointments, travel, communication needs and follow-up actions.
Ask for support finding local services, understanding appointment information and requesting reasonable communication adjustments.
Talk about changes in mood, sleep or anxiety early and agree what support feels helpful before things become urgent.
Build realistic routines around movement, food, hydration, sleep and the activities that help you feel well.
Where agreed in the care plan, staff can support safe routines, records and conversations with prescribers or pharmacists.
Residents can speak with their support team about a concern, ask for a review or request help contacting a health professional.
How SLS can help
A support worker can help prepare questions, arrange reasonable adjustments, attend an appointment where agreed, understand next steps and record actions in the care plan. Clinical advice, diagnosis and treatment remain the responsibility of qualified health professionals.

Need to talk?
Where a current staffing plan provides support around the clock, residents can speak to the on-site team. For a general non-urgent enquiry, contact the SLS office during working hours.
