Tell us what you need
You can contact us yourself. A family member, support worker or someone you trust can help you. Tell us where you want to live and what you would like help with.
A simple guide
A referral means asking us to consider whether SLS could be right for you. You do not have to decide everything before you contact us.
You can ask someone you trust to help at every step.

Our referrals team will talk with you about your wishes and needs. You can tell us how you prefer to communicate. You can ask questions.
We check which homes or support could suit you. We also discuss who will pay for the housing and support. Your council or NHS team may need to be involved.
If there is a suitable option, we can arrange a visit. You can bring someone you trust. Before you decide, we explain the home, the support and what happens next.
