These pathways describe the needs to discuss at assessment. They do not indicate that every service supports every need.

Before a placement is agreed

  1. Discuss the person’s goals, communication, support needs, risks, preferred location and timescale. Send a brief initial enquiry; agree a secure route for supporting documents.
  2. Confirm suitability, current availability, staffing and out-of-hours arrangements, accessibility and any specialist input.
  3. Record the housing and care providers, the tenancy or licence, rent, support costs, funding, transition plan and review arrangements.

Housing and care are separate arrangements. The person’s agreements and support plan identify responsibilities for the specific service.
